After your first visit…
After your first visit with us, you will be sent home with two papers - one for icing instructions and one for stretches. This being said, we know that life happens and it’s not always easy to keep up with papers. Because of this, we have made these instructions available to you at home! Simply click on the buttons you see below and you can download and print new copies.
For Returning Patients…
If it has been over a year since we have seen you, you will need to fill out some update paperwork.
For your convenience, we have created PDF copies of these forms that you can print out at home and bring with you to your re-exam.
You will only need to fill out the information in the hi-lighted brackets. If the information has not changed, please write “N/A”.
